This new, innovative service funded under LHC’s Business 900, has been created to assist service users who may find move on difficult, to live independently with support.

The service aims to develop support strategies, to assist with move-on and to facilitate any support needed post move-on for tenants of LHC who have long term support needs. The service will also provide a point of contact for ex-tenants and external agencies to ensure that the transition to independent living goes smoothly and remains successful.The service is staffed by a full-time Link Worker who will work with the service user and other agencies in a multi disciplinary way to ensure the best package of support is available.



Aims & Objectives

• To assist LHC’s service users who have long term support needs, to obtain and successfully manage their own tenancies.
• To enable vulnerable individuals to achieve their full potential for independence and social inclusion, by working in conjunction with external agencies.
• To develop and implement a volunteer befriending scheme to engage with service users.
• Bridging the gap between support and independence to ensure that more people return to living in the community successfully.



Support Services


The Link Worker will:

• Work with tenants and their key workers to encourage and enable move-on.

• Liaise with external agencies to identify and facilitate appropriate sources of accommodation and support.

• Assist in identifying and securing appropriate tenancies.

• Develop and implement a multi agency support package appropriate to the individual’s needs and wishes post move-on.

• Support and supervise a volunteer scheme, which will befriend service users who are living in their own tenancies.

• Provide a point of contact and an ongoing link, between service users and LHC.

• Monitor the success of ex-tenants post move-on.

• Work in conjunction with other agencies to ensure past tenants are coping successfully in their independent tenancies.



Referrals and Eligibility

The service is available to all who have been identified as at risk of having major problems around move on and sustaining an independent tenancy.

This service is open to individuals who have been tenants of LHC for two years or more, for whom it would be difficult to sustain a tenancy independently in the community. Both LHC staff and service users can make referrals, over the telephone or using a referral form via fax or email. At referral a needs and risk assessment will be carried out to ensure that the referral matches the eligibility criteria.
